MPH and MHA students will have from May 5 - May 22 to review their grades and decide if they would like to request an “S” grade in any of their PBHL courses. This one-time exception applies to the 16-week courses and 2nd 8-week courses in the spring 2020 semester only.
A separate form is required for each course. You will enter the course number and title, and the email address for your faculty advisor and the course instructor. After you submit the completed form, it will auto-route to your instructor to verify that your current course grade meets the parameters set by your program and then to your faculty advisor. It will then auto-route back to Student Services so the change can be processed before the May 29 IUPUI deadline.
If you receive tuition benefits from your employer, before initiating the “S” option, you are encouraged to check with your employer to see if you can be reimbursed for courses in which you receive “S” grades.
